We know social isolation and loneliness can have a big impact on older people and that’s why our community wellbeing services integrate health, housing and care - helping our customers achieve independence and offering a connection to the community - all with the security of 24 hour support available should they need it.
At our community wellbeing schemes we’re able to manage our customers’ health and wellbeing conditions in their own home - preventing unnecessary admissions to hospital and delaying entry into a residential or nursing care setting.
It goes beyond the traditional extra care service and our care is always person-centred. It’s really important to us that our customers feel empowered, maintain and develop their skills and remain close to their families and support networks - all with the reassurance that they can access specialist care if they need it.
Our community wellbeing schemes keep our customers active for longer, less isolated and empowered to do the things they love, while reducing their dependency on primary care by managing their conditions through our service.
